In consideration of being allowed to participate in the lacrosse clinic organized by Taylor Moreno, LLC and Taylor Moreno Lacrosse, I hereby agree to the following:

​I understand that participation in this lacrosse clinic involves physical activity and inherent risks, including but not limited to injury, illness, or accidents. I voluntarily assume all risks associated with my participation.

I represent that I am physically fit and capable of participating in this clinic. I have not been advised otherwise by a qualified medical professional.

I, on behalf of myself, my heirs, executors, and assigns, hereby release and hold harmless Taylor Moreno, LLC, Taylor Moreno Lacrosse, and their respective officers, employees, agents, and volunteers from any and all claims, liabilities, or causes of action arising out of or in any way connected with my participation in the clinic, including but not limited to claims of negligence.

In the event of an emergency, I authorize clinic staff to administer first aid and/or seek medical treatment for me as necessary. I agree to be responsible for any costs associated with such treatment.​​​

I grant permission to Taylor Moreno, LLC and Taylor Moreno Lacrosse to use any photographs, videos, or other media taken during the clinic for promotional purposes, including social media, without compensation.

 I have read and understood this waiver in its entirety. I acknowledge that I am signing it voluntarily and that it is a binding legal document.
